Please complete this form if you would like to receive a COVID-19 vaccination. This places you on our list to receive the vaccine when it becomes available for you. Eligible recipients must be at least 12 years old or older. All must be willing to receive the initial vaccine AND booster vaccine 3 weeks later.

    COVID-19 Vaccination Facts

    Two doses are needed to complete the vaccination. The second shot will be given 3-4 weeks after the first shot. Both doses of the vaccine will be given as a shot in the arm muscle. Precautions (such as wearing a mask, avoiding gatherings, and washing our hands) should still be taken after vaccination.

    • COVID-19 vaccines will not give you COVID-19.
    • COVID-19 vaccines will not cause you to test positive on COVID-19 viral tests.
    • People who have gotten sick with COVID-19 may still benefit from getting vaccinated.
    • Getting vaccinated can help prevent you from getting sick with COVID-19.
    • Receiving the vaccine will not alter your DNA.

    Most people who receive a COVID-19 vaccine experienced mild side effects. Some side effects range from mild to moderate. Possible side effects can include:

    • Injection Site Pain, Swelling, or Redness
    • Tiredness
    • Headache
    • Muscle Pain
    • Chills
    • Fever
    • Nausea
    • Feeling Unwell
    • Swollen Lymph Nodes
    • Joint Pain

    There is a remote chance that the vaccine could cause a severe allergic reaction. Inform your healthcare provider if you have a history of reactions to vaccinations prior to receiving the COVID-19 vaccine. A severe allergic reaction would usually occur within a few minutes to one hour after getting vaccinated. Signs of a severe allergic reaction can include:

    • Difficulty Breathing
    • Swelling of your face and throat
    • A fast heartbeat
    • A rash
    • Dizziness and weakness
